During telehealth consultation:

Details or your medical history, examinations, x-rays, and tests will be discussed with health care professionals through the use of interactive video, audio and telecommunication technology.

A physical examination may take place.

A non-medical technician may be present in the telehealth studio to aid in video transmission.

Video, audio and/or photo recording may be taken of you during the procedure(s) or service(s).

Medical information and records: all existing laws regarding your access to medical information and copies of your medical records apply to this telehealth consultation.

Please note, not all telecommunications are recorded and stored. Additionally, dissemination of any patient images or information for this telehealth interaction to any other parties or entities shall not occur without your consent.

Confidentiality: Reasonable and appropriate efforts have been made to eliminate any confidentiality risks associated with telehealth consultation, and all existing confidentiality protections under state and EU law apply to information disclosed during this telehealth consultation.

Rights: you may withhold or withdraw your consent to the telehealth consultation at any time without affecting your right to future care or treatment.

Risks, consequences and benefits: you have been advised of all the potential risks, consequences and benefits of telehealth. Your healthcare provider has discussed with you the information provided above.

By agreeing to the terms and conditions, you agree to participate in the telehealth consultation.

NON-ASSIGNMENT OF RIGHTS

Your rights or whatever nature cannot be assigned nor transferred to anybody, and any such attempt may result in termination of this Agreement, without liability to us. However, we may assign this Agreement to any person at any time without notice.

NON-WAIVER

Failure of Ausphysio Ltd, trading as Wannarun Chartered Physiotherapy and Running Clinic, to insist upon strict performance of any of the terms, conditions and covenants hereof shall not be deemed a relinquishment or waiver of any rights or remedy that we may have, nor shall it be construed as a waiver of any subsequent breach of the terms, conditions or covenants hereof, which terms, conditions and covenants shall continue to be in full force and effect.
